In November 2009, representatives from every stakeholder in the Dutch oil
and gas industry participated in an Industry Safety Day. Organized under
guidance of NOGEPA, NVDO, IRO and IADC the day was also attended by
representatives from government agencies. During the day, in both discussion
groups and in a panel session at the end of the day, projects were proposed
to improve Safety in our industry. One of the projects proposed was the
development of a standard bridging document.

following own procedures; either because people have not understood the

compliance successful and hereby improve safety performance.

Kuwait Oil Company has planned for a massive expansion of its drilling
activities. This growth is part of the strategic goal and long term policy
for Kuwait to lift its production capacity despite global and economic
challenges. To fulfil its greater objective of enhanced production, a plan
was formulated to enhance its rig fleet. A massive expansion has been
planned in terms of increase in number of rigs deployed. In figures, this
reads as a total growth rate of 80% in just two years. To achieve such
phenomenal growth in such a short time, requires extra-ordinary planning in
terms of HSE and manpower. KOC has proved itself by scripting this success
story in such a short time.

Safety Cases can be large complicated documents which have masses of detail,
some of which may be irrelevant. Normally included in the Safety Case are
such things as Quantitative Risk Assessments (QRA), which mainly only risk
practitioners understand and are of little value to your average rig worker.
IADC has done a lot of good work developing guidelines for producing Safety
Cases that would be accepted by legislators worldwide. This
presentation briefly examines the IADC guidelines and identifies how Safety
Cases could perhaps be put in a more practical format, therefore providing a
document of greater value to the workforce and their employers, while
addressing all legislative requirements.

Within the Drilling Hazard Mitigation group at Weatherford International, a
systematic approach to the QHSSE review process has been undertaken through
the implementation of Service Quality Committee's/Teams, aimed at

efficiency improvements through the implementation of a targeted and
systematic approach to QHSSE management review are illustrated.

The Offshore Mechanical Handling Equipment Committee (OMHEC) is comprised of
members from the United Kingdom, Norway, Denmark, and the Netherlands all of
whom are involved with the safety of lifting equipment and lifting
operations offshore. OMHEC’s objectives are to contribute to improved safety
in offshore lifting operations and to be the arena for harmonizing good
practice in lifting operations offshore in the North Sea area. In this
respect the exchange of information concerning accidents and incidents plays
an important part in the work of OMHEC. OMHEC is also the adviser for the
North Sea Offshore Authorities Forum (NSOAF) in matters related to lifting
offshore, OMHEC’s guidance can be accepted by the regulatory authorities
mentioned above as being good industry practice.

multinational audit on lifting equipment and lifting operations has been
conducted by four of the regulatory bodies of
NSOAF.
The scope of work
was to perform a systematic theme audits on duty holders/operators and
drilling contractors, which are active in two or more NSOAF member sectors.
The presentation will focus on safety challenges and areas for specific
improvement as which come out from the audits. Attention will also be made
to material handling concerns on new MODUs and to recent lifting incidents
in the drilling area resulting in NSOAF Safety Bulletin no. 2010/1.

Panel Discussion: “Third Party Personnel on Rigs – Who ‘Owns’
Them?”

Third party personnel are on all drilling rigs. Although provided by the
operator, they work on the drilling contractor’s facility and under his SMS
and EMS. Is their work controlled to the same level of oversight as drilling

impact? These questions and more will be explored in this panel session.

Drilling Machinery -
HAZOP for Improved Safety

Serious
injuries have resulted from the deficient automation of drill floor
machinery. A new HAZOP based approach to reduce the number of such incidents
is developed by HSE/OSD in the UK. It draws from experience in investigating
drill floor accidents and incidents on the UKCS. The paper outlines how such
a HAZOP study might be implemented. It provides examples of how to robustly
examine drilling machinery control system preventative safety software, and
how the outcome of this study might be used generically with other drilling
rigs.

are often seen as boring documents that sit on a shelf in the OIM's office,
never to be read again. The new IADC format for HSE cases gives the
opportunity, through, amongst other things, the use of Bow Tie analyses, for the workforce to actively participate in their development. Odfjell Drilling involved its workforce in developing the HSE Case for a new
build rig and the presentation also explores the potential uses of Bow Tie
analyses and their relevance for an operational unit.

Step Closer to Zero Incidents by Optimizing Incident Management and Risk
Assessment Processes

How to optimize information flows and risk assessments so as to create a
safer working environment? Obstacles in the information gathering process
are identified for near misses, incidents, accidents or health-related
aspects and investigate how they can be eliminated. It also flags up
efficient ways of assessing health and safety risks and integrating managers
and employees into the HSE process to improve behavioural safety. A case
study of an oil and gas company provides practical examples of how to
minimize risks in occupational health by getting the workforce involved and
establishing an effective EH&S strategy.
