Understanding the Safety - Hazard - Risk Relationship in a Reactive Culture

Safety is defined as a judgment as to the tolerability of RISK, having evolved from a judgment as to the acceptability of RISK. Here we see a definite move from the idealist to the realistic. Policies and Procedures, Law and Regulations, Codes and Standards, etc. dictate levels of acceptable risk (idealistic). The person doing the job ultimately decides what level of risk is tolerable, and as such, defines the level of Safety within the facility or operation (realistic). Our goal is to ensure tolerable never exceeds acceptable and that proper decision making is used to assess both. This paper shows how developing the proper balance of Proactive Controls (Risk Mitigation) and Reactive Recovery (Consequence Mitigation) leads to operational excellence/efficiency.

The ETTO (Efficiency Thoroughness Trade Off) Principle - A Better Way to Understand Safety Performance

The ETTO principle refers to the fact that people always have to make a trade-off between the resources they spend on preparing an activity and the resources they spend on doing it. The trade-off may favour thoroughness over efficiency (if safety and quality are the dominant concerns) and efficiency over thoroughness (if throughput and output are the dominant concerns). It follows from the ETTO principle that it is never possible to maximise efficiency and thoroughness at the same time. This presentation will explain how we can apply the ETTO principle and why only investigating when something goes wrong (incidents) is preventing us understanding performance.

Human Factors Evaluation of Drilling and Completions Equipment

A proactive approach to identify and mitigate hazards to personnel from offshore Drilling and Completions equipment that was developed and implemented for a major oil and gas operator. Hazards were investigated in Hazard Identification (HAZID) workshops that involved a multidisciplinary team comprising of Human Factors, HSE, Engineering and Operations representatives. A task based approach was used which focused on operator interactions with equipment and identified design and operational hazards that were overlooked in previous assessments. The deliverables from the assessments included practical and actionable recommendations to mitigate the hazards. A causal and trends analysis was undertaken to identify the key causes for injuries occurring when interacting with equipment.

Kuwait Oil Company is currently experiencing a massive growth in its drilling activities despite current economical downtime. More than 27 rigs are set to enter service next year, that's about 87% expansion in its current fleet. The company is investing in new fresh college graduates to fulfil the manpower requirement and serve as the company engineering spine. Training was a key factor in achieving this goal and meeting target growth. Previous training models developed those engineers within 2-3 years, whoever, the expansion demand required to develop them in only one year. New tools in coaching, mentoring and competency have been developed. The program was completed with excellent results with more than 85% passed the competency and currently working independently in their parent teams.

The current trend of moving toward Competency-Based Training (CBT) in the Drilling Industry in Australia and beyond will be addressed during this presentation. Well control training is currently provided by both higher education providers and Registered Training Organisations (RTOs) in the Vocational Education and Training (VET) sector in Australia.  The development of the RII09 National Training Package for recognition of experience working in the Drilling Industry has potential for maximising safety outcomes through a paradigm shift toward CBT in the VET sector in general and the Australian Drilling Industry in particular.

HSE Management System is Comprehensive Approach to Reduce The Hand and Fingers Injuries

In an effort to reduce hand and finger injuries, Apexindo established an HSE improvement plan based on Cooper's Reciprocal Safety Culture Model, utilizing a variety of measures from incident analysis, HSE leadership, behavioral factors, inspection, customer interaction, performance recognition program, and auditing of the overall safety climate.  As a result hand and finger injuries were significantly reduced within the company.

Using Technology for Global Incident Reporting & Investigation and Sharing Real Time Information.
